Psychological drama Doctor Foster was a massive hit for the BBC when its first two seasons aired in 2015 and 2017, respectively. Now Suranne Jones (Vigil) is reprising the role that made her famous for a third and final installment, a much-rumored and long-awaited final outing that will doubtless follow up on its devastating Season 2 ending.

In the series’ first season, successful doctor Gemma Foster’s (Jones) life unravels when she suspects her husband of having an affair. What follows is a complicated plot built on paranoia, manipulation, revenge, and desperation that culminated in the disappearance of her then-fifteen-year-old son. Now, nearly a decade later, Gemma is finally on the verge of a fresh start; she’s still living in that same house (why??), but she’s met someone new, and they’re planning to marry. But as the big day draws closer, her complicated past begins to influence her hopeful present.

Alongside Jones, Bertie Carvel (A Knight of the Seven Kingdoms) will reprise his role as Gemma’s ex-husband Simon. Tom Taylor (The Bay) is also back as Tom, now quite a bit older — and taller — with plenty of questions of his own. Also returning from the show’s first two seasons: Anna Madeley (All Creatures Great & Small), Adam James (The Couple Next Door), and Thusitha Jayasundera (Humans).

Suranne Jones, Bertie Carvel, Tom Taylor, Dustin Demri Burns, Adam James, Thusitha Jayasundera, Anna Madeley, and Rosalie Craig will star in 'Doctor Foster' Season 3 (Stuart Bailey/Bertie Carvel/Michael Shelford/George Fox/Dillon Bryden/Wolf Marloh/Harry Livingstone)

Here’s the third season synopsis.

Ten years ago, on discovering her husband Simon was having an affair, Gemma Foster enacted a masterful revenge. But the fallout was devastating when her 15-year-old son Tom disappeared. Now, in Season 3, Gemma is still a GP, still in the same house, but on the brink of a fresh start: she has met someone new and is getting married. But as the wedding day draws closer, and friends and family gather, shadows from the past begin to re-emerge threatening both her happiness and her reputation. As Gemma fights to protect those she loves and expose whoever’s intent on hurting her, will she be able to put the past to bed, dispense justice, and claim the future she deserves, before it is too late?
Gracie Oddie-James, Lois Baglin, Karima McAdams, Anjana Vasan, Michael Balogun, Christina Balmer, Justin Salinger, and Ash Tandon join 'Doctor Foster' Season 3 (Riya Kataria/Kim Hardy/Marco Majocchi/David Reiss/Kate Vice/Nick James/Stuart McClymont/Andy Brown)

New faces joining the cast include Dustin Demri-Burns (Criminal Record), Rosalie Craig (The Serpent Queen), Gracie Oddie-James (Grantchester), Lois Baglin (Surface), Karima McAdams (Hijack), Anjana Vasan (Wicked Little Letters), Michael Balogun (Sherwood), Christina Balmer (Mobland), Ash Tandon (Malpractice), and Justin Salinger (Hanna).

Mike Bartlett (Press) created and wrote the series, with John Hardwick (The Buccaneers) directing and Nige Watson producing. Executive producers include Bartlett, Jones, Roanna Benn, and Jude Liknaitzky.

The show originally aired in the U.S. on the cable channel Lifetime (retitled Doctor Foster: A Woman Scorned). Considering the upheaval of the last decade (including Lifetime’s parent company, A&E, being sold to Hearst), it’s not a surprise that BBC Studios is currently looking to find it a new streaming home.

Doctor Foster Season 3 is expected to debut on BBC One before the end of 2026. It does not have confirmed American distribution.
